AMI Clinic Information

Dear Palisades Community,

Palisades will offer a vaccination clinic (Pfizer vaccine) at two schools on Wednesday, May19th (shot #1) and Tuesday, June 8th (shot #2).  The clinics will be held at Palisades Middle School and Palisades High School on those dates. 

The vaccines are being offered to:

  • All Palisades students (face to face or PCA) ages 12 and older (with parent/guardian permission for students regardless of age)

  • Any Palisades faculty or staff members who have not been vaccinated and wish to receive the Pfizer vaccine

  • Any children in Palisades ages 12 and older who do not attend a Palisades school (with parent/guardian consent regardless of age), as well as any parents/guardians or other community members who wish to receive the Pfizer vaccine

All vaccinations will be coordinated (registration, administration and post injection observation) through AMI by trained medical professionals. The district will manage the process through communication, scheduling during school hours and cross referencing our student registration lists with parental consent for those students receiving their vaccination during school hours.  All students who are receiving a vaccination during school hours must have a parental consent form (regardless of the student’s age). Parents/guardians or other members of the community will not be vaccinated during the school day nor will they be permitted to enter the school vaccination sites during school hours; if you wish to be vaccinated on May 19th, a block of time after school is available at PALMS for vaccinations (see details below).

Only students who are on our registration list and have a parental consent form submitted by noon on Tuesday, May 18th will be vaccinated during school hours on Wednesday, May 19th.  We will use Tuesday afternoon to cross reference our list with parental consent forms. 

Any student may receive a vaccination after school hours (from 2:30PM-4PM) at PALMS on May 19th with an adult present.  The signed parental consent form may be submitted at the time of vaccination after school hours. 

HOURS OF OPERATION ON WEDNESDAY, MAY 19TH:

Palisades High School (Purple Gym) - 8AM - 2PM

  • For Palisades students (12 and up) who are registered to be vaccinated and have submitted the parental/guardian consent form by noon on Tuesday, May 18th.

  • For faculty or staff who are registered to be vaccinated. Faculty and staff can receive a vaccine any time during these hours that are the least disruptive to your work hours (see note below).

Palisades Middle School (Gym follow directions to enter) - 10AM - 2PM

  • For Palisades students (12 and up) who are registered to be vaccinated and have submitted the parental/guardian consent form by noon on Tuesday, May 18th.

  • For faculty or staff who are registered to be vaccinated. Faculty and staff can receive a vaccine any time during these hours that are the least disruptive to your work hours (see note below).

Palisades Middle School (Gym follow directions to enter) - 2:30PM - 4PM after school hours

  • For all students (ages 12 and up) who are registered to be vaccinated; children must be accompanied by an adult and have a completed parent/guardian consent form which can be submitted at the time of vaccination.

  • For faculty or staff who are registered to be vaccinated.

  • Parents/guardians or other community members who are registered to be vaccinated.

NOTE:  Clinic hours on June 8th will be determined after the first round of vaccinations is complete.

ALSO NOTE: Faculty and staff may receive a vaccine any time and at any location noted above provided the time you select is not disruptive to your work hours (select before work/after work, during lunch, prep, etc.) Please let your supervisor know you are getting a vaccination during your work hours in the event you are delayed and/or a brief coverage needs to be arranged. Plan for the process to take approximately 25 minutes for check in, injection and 15 minute observation. Faculty/staff will move to the front of the line during school hours to expedite the process. Please submit a copy of your vaccination card to your school nurse after your second shot. This information will assist our nurses in determining if you need to quarantine following exposure to a COVID positive person.  Due to the voluntary nature of receiving the vaccination, mileage reimbursement will not be provided if you choose to drive to a vaccination site during school hours. 

HOW TO REGISTER FOR THE VACCINE CLINIC:

STEP 1:  Complete a registration for each individual receiving the vaccination; you MUST use a separate email address for each individual you are registering - CLICK HERE TO REGISTER

STEP 2:  If you have registered a student, please review the vaccine information sheet, then download, print and complete the Minor Vaccine Consent Form (attached here). The AMI consent form notes the consent form is for individuals under the age of 18. However, for vaccinations administered during school, Palisades is requiring a consent form regardless of the age of the student.  Scan and email the form to your building nurse - PALMS (Mrs. Harwick email link) or PHS (Mrs. Fulton email link) -students may also remit the form to the nurse’s office.

STEP 3:  Complete this google form - https://forms.gle/nrXzi7rPxHsGW4NM8 - to confirm your registration for students only.

WHAT YOU CAN EXPECT:

  • You/your child will be checked in at the registration desk at the clinic location

  • Please review the INFORMATION SHEET (attached here ) if you have any questions regarding the vaccine

  • After receiving the vaccine, you/your child will be asked to relax in the nearby waiting area for 15 minutes (water and snacks will be available) for observation

  • You will receive verification of your vaccination dose before leaving the clinic

  • You will receive a confirmation email from our clinic provider at the email address used to process the registration
