Please complete this waiver and submit it to your building's facilitator if you complete a volunteer service with an agency that has not completed an application with Palisades School District.
VSE Time Card
Students should use this form to track their completed volunteer service hours. Students are reminded to kindly have the Agency Contact sign off when they complete their service hours in order to receive credit.
